Mattel Children's Hospital at UCLA is located on the University of
California, Los Angeles campus, 10 miles away from the airport and 4 miles from the
Pacific Ocean. In early September, you can expect to enjoy low humidity and
temperatures in the 80s during the day, and mild evenings.
CONFERENCE SITE AND ACCOMMODATIONS: DE NEVE
PLAZA
De Neve Plaza is UCLA's newest conference facility. Everything you might need during your stay is no more than a minute away: the state of the art De Neve Lecture Auditorium where all the lectures will take place, the dining rooms and the student housing accommodations. Rooms have daily maid service and are equipped with telephones, color televisions with cable access, air conditioning, high-speed Internet access (for those with laptops), and private baths. In the dining rooms, you'll have a wide choice of meals (all you can eat). UCLA's full recreation facilities are available complimentary to De Neve Plaza guests. The Sunset Canyon Recreation Center includes two pools and tennis courts. Racquetball courts, basketball courts, and weight rooms are located at the John Wooden Center.
Parking is located at DeNeve Plaza for residential guests
and commuters at a charge of $9 per day.
The residential rates listed below cover accommodations for the nights of Monday, September 1 through Sunday September 7, 2008, breakfast each morning, and access to UCLA's Sunset Canyon Recreation Center and John Wooden Center. (Rates include 14% occupancy tax.)

REFUNDS
A service charge of $50 is withheld from all refunds for the course registration (full refund if course is canceled, discontinued, or rescheduled). Refund requests must be in writing and postmarked no later than August 22, 2008. Mattel Children's Hospital UCLA reserves the right to cancel the course, in which case full refund is granted.

Continuing Medical Education to provide continuing medical education for
physicians.

School of Medicine at UCLA designates this continuing medical education activity
for a maximum of 48 Category 1 credit(s) toward the Physician's
Recognition Award of the American Medical Association. Each physician should
claim only those credits that he/she actually spent in the educational activity.
DISCLOSURE
The FDA has issued a concept paper which classifies
commercial support of scientific and educational programs as promotional unless
it can be affirmed that the program is "truly independent" and free of
commercial influence. In addition to independence, the FDA requires that
non-promotional, commercially supported education be objective, balanced, and
scientifically rigorous. The policy further states that all potential conflicts
of interest of the CME staff and faculty be fully disclosed to the program's
participants. In addition, policy of the Accreditation Council for Continuing
Medical Education now mandates that the provider adequately manages all
identified potential conflicts of interest prior to the program. We, at UCLA
fully endorse the letter and spirit of these concepts.
